TL;DR: If one of the following errors are happening, it's possible that 1Password Desktop is not open or unlocked.

I also saw the following error when trying to connect via ssh using 1Password pinentry:
sign_and_send_pubkey: signing failed for RSA "~/.ssh/id_rsa" from agent: agent refused operation
Solution #
In my case, I noticed this was happening because of two things:
- 1Password Desktop was not open/unlocked, which is expected behavior since I prefer to keep it locked for security.
- I had disabled 1Password's menubar icon, which is required for CLI integration. Re-enabling it resolved the connection issues.
